On April 9, 2024, registration for Pre-K opens. For individual appointments to help register, CALL Zapata NOW at 773-534-1390 to apply for School Year 2024-2025. KINDERGARTEN REGISTRATION
Kindergarten registration will take place Monday-Friday from 8am-3pm in the main office. The child must be 5 years old on or BEFORE September 1, 2024. Must bring: birth certificate, 2 proof of address, parent photo ID, and immunization card. Only the parent or legal guardian can register a child. Call the Zapata main office at 773-534-1390 for more information.
Estaremos matriculando para el kinder de lunes a viernes de 8 am a 3 pm en la oficina de Zapata. El horario de inscripción será de 8:30 a 11:30 horas. El niño/a tiene que tener 5 años antes o para el 1 de septiembre. Se necesita acta de nacimiento, 2 comprobantes de direción, identification con foto del padre y libreta de vacunas. Solamente un padre o guardian legal puede matricular. Llame a la oficina de Zapata al 773-534-1390 para más información.

ENROLLMENT
All children living within the attendance boundaries can be admitted to Zapata Academy at any time throughout the school year.
For information regarding preschool, please visit the Preschool Registration section of the website. If you already applied, you need to take the following documents [1) Birth certificate of child, 2) Parent's proof of income, & 3) Parent's proof of address] to these sites for verification before finishing your enrollment here at Zapata.
Kindergarten Enrollment Eligibility: All children residing in the City of Chicago who are 5 years old on or before September 1 of the current school year, are eligible for enrollment into a CPS Kindergarten program.
Please review the CPS Elementary School Registration Checklist. The parent, legal guardian, or temporary custodian of any elementary age child enrolling in a Chicago Public School is required to submit proof of age, required immunization records, and two proof of address documents to the school.
HEALTH REQUIREMENTS
Families must submit the following medical records prior to the start of the school year: proof of physical examination, proofs of immunization, and dental records according to the CPS enrollment procedures.

- entering preschool, up to age 5 (physical exam and lead screenings)

- Diptheria, Pertussis (Whooping Cough), Tetanus (DTP/Td)
- Inactivated Polio
- Measles
- Rubella
- Mumps
- Hepatitis B
- Varicella (Chicken Pox)
- Haemophillus Influenza, Type B (HIB)
Dental Requirement
All students in Kindergarten, 2nd, and 6th grade must have a dental exam completed by a licensed dentist prior to May 15th of the current school year.
